How to Use a Male External Catheter

Quick answer: A male external catheter collects urine outside the body and drains it into a bag. To use one, clean and dry the skin, fit the collection device according to its instructions, connect it to a leg bag (day) or larger drainage bag (night) without kinks, and keep the bag below bladder level. Check your skin daily and change the device as directed.

Before you start

  • Watch the manufacturer's setup video if one is available. ActivKare YouTube channel
  • Check the fit. AFEX needs at least 1 inch (2.5 cm) of penis length. Ask us if you're unsure which size or system suits you.
  • Trim hair at the base of the penis if needed, rather than shaving.

General setup steps

  1. Wash your hands and clean the penis and surrounding skin with mild soap and water. Dry thoroughly.
  2. Fit the collection device as the instructions describe. For condom catheters, roll the sheath on and leave a small gap at the tip. For non-condom systems, position the pouch or cup and secure the support brief.
  3. Connect the tubing to a leg bag for daytime, or a larger bag for overnight.
  4. Secure the bag below bladder level, with no kinks or loops in the tubing.
  5. Check that it drains after you urinate.

Daily care

  • Empty the leg bag when it's about half to two-thirds full.
  • Check skin every day for redness, swelling or sores.
  • Change or clean components on the schedule the manufacturer recommends.
  • Rinse and air-dry reusable bags, and replace them regularly.

Troubleshooting

Problem Try this
Leaking around the device Check size and positioning; make sure skin is dry when fitting
Not draining Check for kinks; keep the bag lower than the bladder
Skin irritation Check fit; take a break and call your nurse if it continues
Comes off at night Adjust the support brief or ask us about other sizes

Frequently asked questions

How long can you wear a male external catheter?

It depends on the product. AFEX and many condom catheters are changed once a day. Follow the manufacturer's directions for your system.

Can I wear an external catheter if my penis is retracted?

Usually not. Condom catheters are hard to keep in place, and AFEX doesn't work with a retracted penis. Absorbent guards, pads or protective underwear are often a better fit. Your nurse or continence specialist can suggest other options, or call us to talk it through.

What size drainage bag should I use at night?

A larger night bag (often 2,000 mL) avoids overflow while you sleep.
